Notice of Privacy Practices

Effective date: July 15, 2026

Important — practice review required: This is a standard Notice of Privacy Practices template prepared for the practice. Before relying on it, the practice must confirm it matches its actual privacy practices, add its designated Privacy Officer's name and direct contact, confirm the effective date, and have it reviewed by its HIPAA compliance resource or legal counsel. Our commitment to your privacy

America's Health & Wellness Primary Care is required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. We are required to notify you following a breach of unsecured PHI.

How we may use and disclose your health information

For treatment

We use your health information to provide and coordinate your care — for example, sharing information among your providers, or with specialists, labs, and pharmacies involved in your treatment.

For payment

We may use and disclose your health information to obtain payment for services, such as billing your health plan and confirming coverage and benefits.

For health care operations

We may use your health information for our operations, such as quality review, scheduling, staff training, and improving our services.

Others involved in your care

With your permission, we may share relevant information with a family member, friend, or others you identify as involved in your care.

As required or permitted by law

We may use or disclose your information when required by law — for example, for public health activities, reporting abuse or neglect, health oversight, judicial proceedings, law enforcement in limited circumstances, and to avert a serious threat to health or safety.

Uses and disclosures that require your written authorization

Most uses and disclosures of psychotherapy notes (if any), uses and disclosures for marketing, and any sale of PHI require your written authorization. Other uses and disclosures not described in this Notice will be made only with your written authorization, which you may revoke at any time in writing.

Your rights regarding your health information

Our duties

We are required to maintain the privacy of your PHI and to abide by the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for information we already have as well as information we receive in the future. Any revised Notice will be posted in our office and on this website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
